Description

Stress urinary incontinence (SUI) is featured by an involuntary urine leakage occurring as a result of coughing or sneezing which increases the intra-abdominal pressure.
In his talk, Dr. Thomas will focus on treating SUI with abdominoplasty. Urethral slings are considered the gold standard for the surgical treatment of SUI but are associated with their own set of risks and complications. These work on the principle of creating an angle in the mid-urethra so that there is increased resistance to urinary flow when abdominal muscles and the internal urethral sphincter contracts.

Simulation of this angulation is achieved to an extent by a selective use of abdominoplasty which includes tightening of the abdominal wall along with a Mons lift (Pelvic lift) that will tighten the Urogenital diaphragm thus exaggerating the angle and improving the SUI
A combination of complete abdominoplasty with Mons lift can be offered to improve SUI in people who are interested in body shaping. This concept of tension distributed through the fascial continuity is central to the theory of tensegrity in Biomechanics.

On the other hand, Dr. Gupta will focus on treating SUI that has resulted from abdominoplasty. Abdominoplasty is one of the most common cosmetic procedures around the world today. Unfortunately, SUI is also an increasingly prevalent condition given the increasing average age and BMI of our patient population. This talk will help surgeons to understand which patients are at-risk of worsening SUI post-abdominoplasty, how to diagnose SUI, and how to treat it.

Learning Objectives

Upon completion of the activity, the learner should be able to:

  1. Describe the pathophysiology of stress urinary incontinence (SUI).
  2. Evaluate the role of abdominoplasty and pelvic lift on post parous women having SUI.
  3. Explain the role of Pelvic fascia in modifying the urethral angle and an improved abdominal tone in achieving continence.
  4. Recognize patients who have developed SUI after their abdominoplasty.
  5. Propose symptom management techniques to patients who develop SUI incontinence after their abdominoplasty.

Target Audience

Practitioners in the field of cosmetic surgery including, but not limited to, dermatology, general surgery, and plastic/reconstructive surgery.

Dr. Mohan ThomasMohan Thomas, MD, DDS, FAACS specializes in general cosmetic and facial plastic surgery. Triple board certified, he is the founder and medical director of Dr. Mohan Thomas Aesthetics. He has taught surgery, advanced cosmetic surgery, facial plastics and reconstruction surgery at universities and institutions in New York, NY, and Mumbai, India. An esteemed author, he has published more than 50 articles in international journals, and more than 30 chapters in 10 reference books. He also served as chief editor of the six-volume Manual of Cosmetic Surgery.

Dr. Aneesh GuptaAneesh Gupta, MD, MPH is a quadruple board-certified cosmetic surgeon offering full-body solutions at Jazzi Cosmetic Surgery in Philadelphia, PA. Dr. Gupta completed his fellowship in facial and body Cosmetic Surgery at Marvel Cosmetic Surgery in Nashville, TN, with Dr. Jeffrey Marvel.
